· $9,000 to $15,000 Sign on Bonus (depending on position)

RN Field Case Manager for home care. Territories available based on where you live

What you will be doing:

• Field RN Case Manager providing home health visits for skilled nursing care
• Admissions, OASIS, create plan of care, care coordination, re-certifications, discharges
• Provide supervision, skilled nursing care
• Ensure documentation is completed

Skills you need:

• Home Health experience highly preferred
• NY state RN license
