- Collaborating with inpatient facilities and case management staff
- Conducting telephonic outreach and discharge planning evaluations
- Providing support for home health services
- Utilizing critical thinking skills for effective problem-solving
- Ensuring compliance with laws, regulations, and agency policies
- Demonstrating proficiency in interviewing and physical assessment
Skills You Need:
Associates in Nursing; Baccalaureate Degree preferred
Minimum one year of acute, Medical-Surgical nursing experience
Discharge Planning/Utilization Management experience preferred
Proficiency in Word and Excel; current CPR certification
