Job Details

Our Managed Care Health plan is expanding and we are looking to hire several REMOTE RN Case Managers who have current or previous experience working in the Health insurance or Managed care setting.

RN Must have proficient knowledge of Medicaid or Medicare and Managed Care

This role REQUIRES 6 months training in the NYC office before it goes REMOTE - RN must live within 90 miles of NYC and be committed to 6 months training in NYC office.

The RN Case manager will serve as a patient advocate in facilitating, coordinating and communicating the transition of patients to next level of care in accordance with applicable law, regulations and agency policies and procedures.

RNCM will work in a collaborative role with and select inpatient facilities to evaluate inpatients referred for transitional planning and/or home health services. The daily functions are carried out through a combination of telephonic outreach and collaborative efforts with facility discharge planners, and in-house case management staff. managed care nurse, managed care rn, nursing, rn, nurse, care management

Qualifications:

Associates in Nursing; Baccalaureate Degree preferred.
Minimum of one year of acute, Medical-Surgical nursing experience. CHHA/LTHHCP experience preferred.
Discharge Planning/Utilization Management experience preferred.
