Home Care RN Case Manager seeing patients in their homes.

What you will be doing:
· Provide case management services for homecare patients
· Complete initial assessments, OASIS, recertifications
· Creates and implements a plan of care
· Collaborates with the interdisciplinary team and coordinates patient care
· Educates patients and families

Skills you need:
· RN license in NY
· Fluent in Mandarin or Cantonese or Spanish
