Nurse Practitioner- Urgent Care

• Provide comprehensive care for patients with acute and chronic illnesses or injuries in our urgent care setting.
• Collaborate with medical assistants, nursing, other mid-level providers, physicians, and care coordinators to ensure high-quality patient care.
• Conduct assessments, diagnoses, treatment, management, education, and health promotion for patients and their families.
• Perform minor surgical procedures like incision and drainage, laceration repair, and suture removal.
• Support transitions in care, including hospital and ER follow-ups.
• Participate in care coordination and contribute to the Primary Care Medical Home model.
• Ensure accurate and timely clinical documentation in compliance with organizational standards.

Skills You Need

• Accredited nursing or physician assistant degree with completed nurse practitioner program.
• Valid State of Connecticut licensure, with DEA and state narcotic registration eligibility.
• Experience in urgent care or ambulatory settings preferred.
• Strong communication and collaboration skills to work effectively with a diverse team and patient population.
