Job Details
Looking to hire several LPNs who come with at least 2+ years nursing expereince in either Hospice, palliative care or oncology to work in a field based position in Manhattan, where you will work under the direction of an RN Hospice Case Manager.
LPN will visit hospice patients in their home or in their facility setting.
LPN provides direct, skilled nursing care to all patients in need of hospice care and documents each visit using the Netsmart EMR system;
LPN will report any changes in the patient's condition to the RN Case Manager.
