Job Details:

We are looking for a dynamic and experienced Home Health Nurse Clinical Manager to join our team. This is a permanent role that offers the opportunity to utilize your clinical expertise and leadership skills in a unique and rewarding environment. As a Clinical Manager, you will be responsible for the overall supervision and coordination of clinical services. You will monitor compliance, assess patient care, and provide clinical direction to ensure that our patients receive the highest quality healthcare services.

Responsibilities:

1. Manage and oversee the daily operations of the home health care team, ensuring the delivery of high-quality patient care.
2. Coordinate patient care, from admission to discharge, ensuring all care plans are accurately implemented and followed.
3. Monitor patient progress, conduct regular patient assessments, and adjust care plans as necessary.
4. Provide clinical direction and guidance to nursing staff, ensuring adherence to all relevant regulations and standards.
5. Collaborate with multidisciplinary team members to ensure comprehensive patient care.
6. Develop and implement policies, procedures, and protocols to improve patient care and outcomes.
7. Participate in the recruitment, training, and development of nursing staff.
8. Maintain communication with patients, families, and physicians to ensure the continuity and consistency of care.
9. Ensure compliance with all federal, state, and local regulations, as well as with organizational policies and procedures.
10. Participate in quality improvement initiatives and implement strategies to improve patient satisfaction and outcomes.

Qualifications:

1. Current licensure as a Registered Nurse (RN) in the state of Pennsylvania.
2. A minimum of 5 years of clinical nursing experience, with a focus on progressive care and wound care.
3. Previous experience in a managerial or supervisory role in a home health setting is preferred.
4. Strong clinical knowledge, particularly in areas such as progressive care and wound care.
5. Excellent leadership and team management skills, with the ability to motivate and mentor a team.
6. Strong communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare professionals.
7. Proficiency in electronic medical records systems and clinical software.
8. Ability to work independently, make sound decisions, and handle multiple priorities in a fast-paced environment.
9. Commitment to providing the highest quality care and to improving patient outcomes.
10. Ability to comply with all local, state, and federal regulations, as well as organizational policies and procedures.
