Job Details
In this full time Home Care role the RN will provide advanced clinical services and emotional support to patients recovering from illness, injury, or surgery.
You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education regarding the disease process, recovery process.
You will coordinate, implement and continually evaluate the home care needs of your patients and will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers
This is a Field based position Monday thru Friday 830 am to 5pm
Qualifications:
Current NYS RN license and registration required
Bachelor’s degree in nursing preferred; Associate's degree required
Minimum of one year acute medical-surgical nursing experience required
Certified Home Health Agency (CHHA) experience preferred
Bilingual English and Spanish or Mandarin or Cantonese preferred but not required
