Job Details
We are looking to hire several experienced RNs who have at least 2+ years nursing experience in eithe rMedical Surgical, Oncology or home care / hospice nursing to do patient visits in both home and facility settings. The Home Care RN will provide advanced clinical services and emotional support to patients recovering from illness, injury, or surgery.
The Home Care Nurse will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education. You will coordinate, implement and continually evaluate the home care needs of your patients
You will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers
Qualifications:
Certified Home Health Agency (CHHA) experience preferred but not required
Bilingual English and Spanish or Mandarin or Cantonese preferred but not required
